DECLARATION OF COMMITMENT
I hereby make an application for the mentioned child in our care, to be enrolled at Rise UP Academy. In making this application, I / We agree to:

•Understand the Designated Special Character Statement of Rise UP Academy. I / We agree that my child will participate in whole school and class devotions.

•Understand that the Building Learning Communities Whanau programmes the school delivers are to support parents to become fully engaged with their children’s learning. We agree to attend all sessions and complete the Hearts and Minds programme when offered to us.

•Make certain my child attends school regularly, arrives to school on time and in the appropriate uniform.

•Attend all 3 Way, Anniversary and Interim Conferences and IEP’s, to communicate regularly with my child’s teacher to ensure his/her academic success.
